Unless you have caused an injury as a result of a motor vehicle accident, you do NOT have to complete an Accident Report Form.

This form is to be filled out only by a person who is responsible for the accident. If you are responsible for injuring someone as a result of a motor vehicle accident, you must complete this form and return it to Allianz.

Despite what you might be told, you are NOT obligated to complete this form.

If you complete and sign the form you may be giving the Insurer important information which may be used against you by the Insurer to reduce or defeat your claim. In addition you will be giving the Insurer authority to contact and obtain information from even your employers, doctors, hospitals and all State and Federal Government Departments (including the Police, Medicare and Centrelink).
